Scheduling missed lessons and make-up lessons
The school office must be notified of any lesson cancellations before 7 pm prior to the scheduled lesson date in order to qualify for make-up lessons. There are no make-ups for no-show students. Emergency cancellations may be qualified with proof of emergency: If a student calls in sick prior to the lesson on the lesson day, a doctor's note is required for a makeup. If a permanent schedule change is required, please inform the Opus119 school office at least 2 weeks in advance to fill out the Change of Schedule request form. Every effort will be made to accommodate date and time. It is the parent's responsibility to initiate the scheduling of make-up lessons. Make-up lessons are based on teacher's availability. All make up lessons must be done within 60 calendar days from the date of the cancelled lesson. No refunds are given for missed lessons.

Credits
Credits: 2 weeks notices are required for any credits due to vacation for more than 2 weeks. Credits will be issued towards the 2nd billing cycle.
Group Lessons/Theory Class:
Group Lessons: There are no make-up lessons available for group lessons. No refund or credit for missed lessons.
Theory Class: Make up class will be given to missed theory class, cancellation policy is the same as instrumental cancellation policy. There are no credits or refunds for missed classes. There are no makeups or credits for missed CM workshops.

Holding lesson times
Holding lesson times: Opus119 can only hold lesson time slot for 2 consecutive weeks of absence.
Lesson Holding Policy Rider: Opus119's lesson policy states that if a student is absent from the lesson schedule more than 2 weeks, the school will not be able to guarantee the lesson spot. This policy is a rider to hold the lesson spot for the students who will want to secure the lesson spot during a longer absent from the school.
*Lesson holding fee: $15 for every half hour spot starting on the 3rd week.
*Maximum lesson holding days: 30 days

Group Lessons: There are no make-up lessons available for group or Theory classes. The school will not give refund or credit for missed lessons.
Substitutions:
The school reserves the right to provide a qualified substitute teacher if the regularly scheduled teacher is ill or otherwise unable to attend a lesson. If the school cannot arrange a substitute, the missed lesson will be made up.

Withdraw
There is a two-month minimum for all lessons. 30-day notice is required to discontinue any classes. Less than 30 days notice to withdraw is subject to a $100 early termination fee. All withdrawals must be submitted in person to office staff and fill out online exit interview form. Withdrawals will not be accepted over the phone. Teachers do not process withdraw requests. Any requested refunds to credit cards outside of lesson crediting will be subjected to a $25 handling fee.
To withdraw from lessons a parent or adult student must:
1. Inform school administration according to the policy procedure.
2. Complete the withdrawal form online.
Automatic bank debiting or credit card charges will stop after the 14-day notice period. Opus119 reserves the right to terminate any lessons to students. If such an event should occur, unused lesson tuition will be refunded. Please inform the school immediately of any credit card number changes, address and/or telephone number changes. Please give us a 14-day notice to cancel automatic payments, any shorter notice will result in a $25 handling fee.

Photo/Video/Audio Release
The school is hereby granted permission to take photographs, audio recording and video recording of the students to use in brochures, web sites, posters, advertisements and other promotional materials the school creates. Permission is also hereby granted for the school to copyright such photographs in its name.
